    Chronic fentanyl administration induces some behavioral changes in mice. Although further studies are needed to clarify the underlying mechanisms of the behavioral effects of chronic fentanyl administration, our findings suggest that fentanyl is safe under properly controlled conditions.

    This case report describes a patient entering early remission for OUD when treated with SROM following unsuccessful past treatment attempts on first-line oral medications. Future studies should seek to evaluate SROM-based OAT as a potential second-line treatment alternative for OUD.

    Fentanyl is highly effective in controlling pain in patients with severe renal colic referring to the emergency department. Intranasal administration of fentanyl combination with ketorolac can be an appropriate, non-invasive, easy-to-use and fast alternative to the intravenous method to manage pain in these patients.

    CNS penetrant µ-opioid receptor (mOR) agonists and partial agonists like morphine, codeine, fentanyl, oxycodone and buprenorphine play a key role in acute and chronic pain management...These compounds range from full agonists like RM-1131, that binds the mOR receptor with high affinity (mOR [3H]-DAMGO competition IC50 = 1.8 nM), display full activation of the receptor in vitro in [35S]-GTPγS and cAMP assays and display strong analgesic effects in vivo, to antagonists like RM-1046 that also bind to the receptor with high affinity (mOR [3H]-DAMGO competition IC50 = 7.5 nM) and antagonize mOR agonists in vitro and in vivo. The correlation between in vitro activation of mOR and relative activity in models of analgesia and respiratory depression will be discussed.

    A screen of a virtual library yielded potent and selective mOR ligands typified by the previously disclosed compound, PZM21In this report, we discuss the use of urea and phenol isosteres to optimize the CNS permeability of the PZM21 scaffold. Our efforts led to a novel series of carbamate derivatives typified by RM201006 (mOR cAMP EC50 (%Effic.) = 35 nM (80%) with significantly improved CNS permeability in rat (brain/plasma ratio = 3.2) and mouse (brain/plasma ratio = 4.1).

    A significant part of the problem comes from the proclivity of mOR agonists that are currently prescribed to manage pain like morphine, codeine, fentanyl and oxycodone, to create tolerance and dependence resulting in respiratory depression that can lead to hypoxia and death...It was found that cyclopropane carboxamides with a trans configuration across the cyclopropane ring like RM296 (mOR 3H-DAMGO competition IC50 = 1.4 nM; mOR [35S]-GTPγS EC50 (%Effic.) = 2.1 nM (64)) are full agonists while analogs with syn configuration, typified by RM964 (mOR 3H-DAMGO competition IC50 = 80 nM; mOR DAMGO cAMP IC50 = 48 nM), are antagonists or low-efficacy partial agonists. The in vitro and in vivo activity of these compounds will be discussed.

    In contrast to these effects of antagonists, several opioid agonists (morphine, fentanyl, buprenorphine, and methadone) lacked detectable effects on ERES levels...Me-Ntx failed to significantly increase ERES levels, suggesting that antagonists must enter the cell to serve as pharmacological chaperones, consistent with our proposed inside-out pharmacology mechanism. The possibility that antagonists induce supersensitivity by pharmacologically chaperoning opioid receptors could suggest innovative approaches for opioid abuse disorder.

    Opioids, such as morphine and fentanyl, are widely used for the treatment of severe pain; however, prolonged treatment with these drugs leads to the development of tolerance and can lead to opioid use disorder...Our data also challenge the dogma that shorter forms of β-endorphin do not exhibit receptor activity; we show that they exhibit robust signaling in cultured cells and in an acute brain slice preparation. Collectively, this information lays the groundwork for improved understanding of the endogenous opioid system that will help in developing more effective treatments for pain and addiction.

    We describe 2 cases where telemedicine was combined with street outreach to connect patients experiencing homelessness with OUD to treatment. These cases highlight an important opportunity to provide access to life-saving OUD treatment for vulnerable patients in the setting of a pandemic that mandates reduced face-to-face clinical interactions.

    There is no established strategy for selecting a method of anesthesia in patients with morbid obesity (body mass index 40 kg/m or more). For this reason and considering the amount of bupivacaine used for spinal anesthesia, we wanted to share our experience with spinal anesthesia for cesarean section in a super morbidly obese parturients.

    There was no statistically significant difference (p=0.540) in survival between patients with and without delirium, based on the survival analysis and length of hospitalization. The prevalence of death was 21%, and age, electrolyte disturbance, glycemic index, tube feeding, mechanical ventilation, sedation with fentanyl, use of insulin, and higher Charlson score were associated with mortality.

    Journal:  Determining spatial access to opioid use disorder treatment and emergency medical services in New Hampshire. (Pubmed Central) -  Aug 15, 2020   
    When comparing the access results with data on the towns of residence for individuals who died from unintentional overdoses involving fentanyl during 2015 and 2016, estimates found that approximately 40% of the towns were not estimated to be in the highest class of access to treatment services at the time. This research provides information for local public health officials to support planning strategies to address opioid use disorder treatment access in high-risk regions.
