    A 23-year-old female patient had an uneventful surgery for replacement of a malfunctioning gastrojejunal feeding tube under general anesthesia using Midazolam, Propofol, Fentanyl and Rocuronium. Patient was reversed with Sugammadex before extubation and presented severe muscle spasms after extubation, requiring large doses of midazolam and Propofol, with reintubation and admission to ICU for airway and exacerbation control.

While further investigation is needed to determine the feasibility and efficacy of this technique, success could obviate the need for costly overnight hospital admissions and reduce opioid consumption in the setting of its well-known side-effect profile.

    This is higher than one study reporting mean intraoperative and postoperative amount of morphine of 7.1 mg and 1.8 mg, respectively 2 ; however, in that study, it is unclear as to whether these reported opioid requirements also included the 1 mg/kg morphine and 1 mcg/kg fentanyl used for all patients on induction...4,11 All three patients reported satisfaction with their analgesia, suggesting that the PENG block may have a potential role in reducing pain, minimizing narcotic usage, and decreasing postoperative length of stay in young adult patients. While further studies should be performed to assess the effectiveness of the PENG block, these results continue to support the use of PENG blocks in hip arthroplasties.

The 20% reduction in opioid consumption in the PACU and 39% in the opioid requirement at 24 hours postoperatively were not statistically significant but could however be clinically interesting.

This is the first study to determine the occurrence of NPSs including synthetic opioids, synthetic cannabinoids, synthetic cathinones, and piperazines in the U.S. communities.
