    There were no major procedure-related complications that required intervention. Early adhesiolysis and drainage of fluid using medical thoracoscopy should be considered in patients with multiloculated complicated PPE after careful radiological (ultrasonography and CT) stratification, as a more cost-effective and safe method of management.

This presentation will highlight how NIH researchers are using scientific advances to address the opioid crisis amidst the COVID pandemic, which includes the development of new medications and formulations to help treat opioid use disorders and overdoses; prevention strategies to mitigate an individual’s vulnerability to addiction; and implementation science to guide optimal deployment of therapeutic interventions including the use of telehealth in diverse settings (healthcare, justice setting, rural communities).

    The newly designed fentanyl derivative [( ±)-N-(3-fluoro-1-phenethylpiperidine-4-yl)-N-phenyl propionamide] (NFEPP) was recently shown to produce analgesia selectively via peripheral mu-opioid receptors (MOR) at acidic pH in rat inflamed tissues...These data demonstrate the increased affinity of NFEPP for murine MOR at low pH, and its ability to inhibit bone cancer-induced hyperalgesia through peripheral MOR. In mice, central opioid receptors may be activated by ten-fold higher doses of NFEPP.

    Overall, compared with the control group for knee arthroscopy, fentanyl supplementation is associated with decreased time for sensory block regression to S1 (mean difference (MD) = -47.38; 95% confidence interval (CI) = -56.74 to -38.02; p < 0.00001), first ambulation (MD = -41.65; 95% CI = -65.11 to -18.19; p = 0.0005), first urination (MD = -23.45; 95% CI = -32.16 to -14.74; p < 0.00001) and hospital discharge (MD = -29.39; 95% CI = -44.73 to -14.06; p = 0.0002) but has no substantial influence on onset time of anaesthesia (MD = 0.50; 95% CI = -1.71 to 2.70; p = 0.66), duration for motor blockade (MD = -42.56; 95% CI = -119.18 to 34.07; p = 0.28), pruritus (risk ratio (RR) = 2.17; 95% CI = 0.28 to 16.90; p = 0.46) or nausea (RR = 0.42; 95% CI = 0.10 to 1.81; p = 0.25). Fentanyl supplementation benefits postoperative recovery after knee arthroscopy.

These findings suggest that locomotor muscle group III/IV afferent feedback in HFrEF leads to increased systemic vascular resistance, which constrains stroke volume, cardiac output, and O delivery thereby impairing V̇O peak and thus exercise capacity.

Adjunctive pain management with ketamine infusion, lidocaine infusion, or dronabinol should be considered in patients undergoing procedures with significant post-procedural pain.

    The fentanyl consumption in Group Pv was significantly lower in comparison to Group C in all time intervals (p < 0.05)...Eight patients in Group C, two patients in Group Pi and 1 patient in Group Pv required additional analgesics and the difference was significant (p < 0.05). Paravertebral block achieved more effective analgesia by reducing postoperative opioid consumption and VAS scores comparison to the control and peritubal infiltration groups in patients undergoing percutaneous nephrolithotomy.

Conclusion Preoxygenation in 25° head-up position with 5 cm H2O continuous positive airway pressure significantly prolongs safe apnea period in non-obese adults compared to supine position, with or without 5 cmH2O continuous positive airway pressure.
