    While prescription opioid and heroin involved death rates decreased from 2017 to 2018, synthetic opioid involved death rates (not including methadone) increased by 10%1...Harm reduction strategies include those such as opioid overdose education and naloxone kit distribution, fentanyl test strip distribution, syringe exchange programs, HIV pre-exposure prophylaxis (PrEP), and Good Samaritan/liability laws...This session is designed to provide education on identifying harm reduction strategies, the ethics and legality surrounding the use of harm reduction strategies, and applying evidence-based interventions to patients with opioid use disorder. Learning Objectives: Assess the utility of harm reduction strategies for patients with opioid use disorder (OUD).Advocate for harm reduction strategies for patients with OUD based on ethical and legal considerations.Propose strategies to implement evidence-based harm reduction programs for patients with OUD.

    Application of CTAP, a selective mu opioid receptor antagonist, directly into the KF/PB complex reversed and prevented fentanyl-induced apnea by increasing the frequency of apneustic-like bursting. These results demonstrate that countering opioid effects in the KF/PB complex is sufficient to restore phasic respiratory output at a rate similar to pre-fentanyl conditions, which could be beneficial in opioid overdose.

    Implementation of a comprehensive colon preparation quality intervention resulted in significantly improved inpatient colon preparation quality and decreased frequency of inadequate preparations.     In dorsal root ganglion (DRG) neurons cultured from rats primed with fentanyl, robust nociceptor population-specific changes in sensitization by prostaglandin E (PGE) were observed, when compared with nociceptors from opioid naive rats. In DRG neurons cultured from rats with OIHP, enhanced PGE-induced sensitization was observed in vitro, with differences identified in non-peptidergic [strongly isolectin B4 (IB4)-positive] and peptidergic [weakly IB4-positive (IB4+) and IB4-negative (IB4-)] nociceptors.

    The final, optimized surface-wipe method resulted in an average collection and extraction efficiency (±SD) of 62.0 (±14.0)%, with a range of 34.1 (±2.6) - 82.5 (±9.6)%. While legislation has yet to be implemented regarding remediation levels for fentanyl-contaminated locations, when such legislation is drafted, this method can be implemented to determine the safety of these locations prior to and after decontamination has occurred.

The relevance of NOP receptors and N/OFQ in leukocytes, and of all opioid receptors, opioid peptides and N/OFQ in native glia for pain control is yet to be clarified.

    Over a third of pediatric hospitalizations were associated with the administration of prescription analgesics that have not been labeled for use in children.     The error, they said, was caused by miscoding deaths involving illicitly manufactured fentanyl (IMF) as deaths involving prescribed fentanyl...Except for heroin, methadone, and opium, each of which has an individual program code, all other opioids are separated in just two program codes according to whether they are synthetic or semisynthetic/opiates...The authors conclude that the CDC was at fault for failing to recognize and correct this problem before 2016. Public policy consequences of this failure are briefly mentioned.

    Introduction: Initiating buprenorphine for opioid use disorder can be difficult because of the risk of precipitating withdrawal in patients transitioning from methadone, requiring opioid analgesia, or with regular exposure to fentanyl. A novel method of using intravenous buprenorphine micro-dosing and as-needed full-agonist opioids was successfully used to transition one patient from methadone maintenance and one patient from full-opioid agonist analgesia without precipitating withdrawal.

    Patients who inject methamphetamine were more likely to have a history of drug overdose, HCV infection, fentanyl use, and sex work, but overall, we found similarly elevated prevalence of adverse social determinants of health as patients who do not inject methamphetamine. Patients who inject methamphetamine may particularly benefit from increased access to harm reduction interventions such as naloxone to mitigate opioid overdose risk, safe sex and sterile injection supplies, and supervised injection facilities.
