    A case is described where a property was used as a clandestine opioid (fentanyl and carfentanil) processing, handling, pill pressing, and packaging site...Regulatory hurdles and key lessons are presented so that others may consider development of similar public health policy. Involvement of public health departments in the assessment and cleanup of sites contaminated with opioids and other illicit drugs and the development of standards for cleanup and worker protection is an important and yet often overlooked public health measure.

    Model projections suggest that both addictions and overdoses related to heroin and fentanyl will continue to increase in the next few years (2020-2022), even as addiction to prescription drugs continues to fall. Finally, management strategy analysis suggests that in the changing face of the epidemic, the most successful approach will target availability of treatment with subsequent monitoring of stably recovered individuals to see that they do not relapse, coincident with direct efforts to decrease opioid overdose fatalities (e.g., further availability of Naloxone).

    In addition, the emergence of highly potent synthetic opioids such as fentanyl and carfentanil increases the need to protect workers who may encounter unknown drug substances during job activities...Additional studies are needed to overcome gaps in technical knowledge about personal protective equipment, decontamination, and control methods, and gaps in understanding how these measures are utilized by workers. Increasing our knowledge of how to protect against exposure to illicit opioids has the potential to improve occupational health across communities.

    Less midazolam was present after 180 minutes when given with morphine compared to midazolam given alone in the absence of an oxygenator (1264.9 +/- 425.6 vs. 2124 +/- 254 ng/ml, P = 0.037). We successfully characterized the adsorption of various combinations of midazolam, fentanyl, and morphine to our CPB circuit, showing that fentanyl and midazolam behave differently based on other medications present.

    Our findings highlight high rates of ongoing fentanyl use despite the use of OAT and support the expansion of BUP/NX for the treatment of people who use fentanyl. This pilot program demonstrated the feasibility of using three technologies to assess for the presence of fentanyl in user-submitted samples, revealed discordant results from the technologies, and provided information on the local drug market.

    Our study shows that cyclopropylfentanyl produces typical opioid-like effects in male rats. Cyclopropylfentanyl displays much greater analgesic potency when compared to morphine, suggesting that cyclopropylfentanyl poses increased overdose risk for unsuspecting users.

    TetM0 is the most potent receptor with K ≥10  M toward methamphetamine, fentanyl, MDMA and mephedrone. We report the synthesis of two new acyclic sulfated acyclic CB[n]-type receptors ( TriM0  and  Me 4 TetM0 ) and investigations of their binding properties toward a panel of drugs of abuse ( 1  -  13 ) by a combination of  1 H NMR spectroscopy and isothermal titration calorimetry.   TetM0  is the most potent receptor with K a  ≥ 10 6  M -1  toward methamphetamine, fentanyl, MDMA and mephedrone.   TetM0  is not cytotoxic toward HepG2 and HEK 293 cells below 100  m M according to MTS metabolic and adenylate kinase release assays and is well tolerated  in vivo  when dosed at 46 mg kg -1 .   TetM0  does not inhibit the   hERG ion channel and is not mutagenic based on the Ames fluctuation test.  Finally,  in vivo  efficacy studies show that the hyperlocomotion of mice treated with methamphetamine can be greatly reduced by treatment with  TetM0  up to 5 minutes later.   TetM0  has potential as a broad spectrum  in vivo  sequestrant for drugs of abuse.

    Fifty µg of fentanyl as an adjuvant to intraarticular bupivacaine produces effective and safe analgesia after knee arthroscopy as 100 µg of dexmedetomidine and has a longer analgesia duration in the first postoperative 24 hours. Exposure to opioids for any reason after the diagnosis of unintentional dural puncture is not associated with a reduced risk of PDPH and does not decrease the need for therapeutic EBP.

    However, there was very little evidence to indicate whether NSAIDs affect the rate of breast hematoma or bleeding from any location within 90 days of breast surgery, the need for blood transfusion and incidence of other side effects compared to placebo or other analgesics. High-quality large-scale RCTs are required before definitive conclusions can be made.
