Compared with children of mothers with epilepsy who did not use ASM during pregnancy, we found an increased risk of psychiatric disorders with prenatal exposure to valproate (aHR=1.85, 95% CI: 1.62–2.13) and topiramate (aHR= 1.43, 1.00–2.05), but not with lamotrigine, levetiracetam, carbamazepine, oxcarbazepine, clonazepam, and gabapentin. In pregnant women with epilepsy, treatment with valproate and topiramate was associated with an increased risk of early-onset psychiatric disorders in the child.

Although the pathological correlation between vertigo and epilepsy remains elusive, our findings indicate that vestibular cortical neurons may participate in periodic epileptiform discharges of the frontal lobe. Clinicians should be aware of a potential diagnosis of epilepsy in patients presenting with vertigo as the onset symptom because this condition is usually underdiagnosed.

We reviewed cases of LEV-induced rhabdomyolysis reported in the literature over the last decade and found 13 cases with almost similar correlation between initiation of LEV and increase in CK levels. CONCLUSIONS Our case report stresses the importance of close monitoring of CK levels and kidney functions after initiation of LEV, and to consider changing the anticonvulsant medication if CK levels are noted to be significantly high to avoid kidney injury.

Methods Eligible patients (≥18 years of age) had MM per International Myeloma Working Group criteria, measurable disease, Eastern Cooperative Oncology Group performance status ≤1, progressive MM after 1−3 prior lines of therapy (including a proteasome inhibitor and immunomodulatory drug), and were lenalidomide refractory (no prior BCMA-targeting agent)...Patients received cilta-cel (0.75×106 [range: 0.5–1.0×106] CAR+ viable T cells/kg) as a single infusion 5–7 days after start of lymphodepletion (cyclophosphamide 300 mg/m2 + fludarabine 30 mg/m2 daily for 3 days)...Management strategies included evaluation of infectious and paraneoplastic etiologies upon observation of ICANS ≥ grade 1, administration of tocilizumab (if concurrent cytokine release syndrome [CRS], all grade of ICANS) and/or dexamethasone (grade 2/3) or methylprednisolone (grade 4)...Two of the 3 patients received supportive measures to treat ICANS, including levetiracetam and steroids; all 3 had concurrent CRS and all recovered...With a median follow-up of 5.8 months, there were no movement or neurocognitive disorders in patients with progressive MM who had received 1−3 prior lines of therapy (Cohort A). These results suggest that early detection and management of neurologic AEs can lead to better treatment outcomes.

This study, which, to our knowledge, is the first examining levetiracetam effect on only the first dose of HDMTX, supports the larger retrospective study finding no significant effect of levetiracetam on HDMTX clearance time, and suggests that administering concomitant levetiracetam does not affect HDMTX toxicity.
