    Intraoperatively she received dexamethasone 10 mg IV x 2 (first dose early in case), 5% albumin 100g, lactated ringers 1L, normal saline 1L, and sodium bicarbonate 50 mEq (one ampule) without significant improvement (repeat ABG 7.16/23)...Though surgery is known to trigger euDKA, to our knowledge such severe perioperative acidosis from euDKA in a non-diabetic patient has not been previously reported. Intensivists and anesthesiologists should recognize euDKA as a potential cause of severe perioperative acidosis in non-diabetics with risk factors.

    In our institution we switched the immunomodulatory treatment of COVID from Tocilizumab to Baricitinib when the former became unavailable in late 2021...All studied patients received at least 10 days of steroids (most commonly dexamethasone), with half of the patients receiving additional doses for treatment of suspected organizing pneumonia...Which the two seminal large trials of the medication for this indication show both a low, and no added risk for infection, our experience suggests that the actual risk of infection in a real-world setting may be much higher. Confirmation of these findings requires an adequate case-controlled study (in progress) and expansion to multiple sites.

We believe that this case adds to the existing literature to shed light on the significant morbidity and mortality this infection can have on patients with COVID-19.

All three patients whom have received concurrent therapy with Remdesivir, Hydrocortisone and Monoclonal Antibodies has survived, only one patient has died as a direct complication of COVID-19 ARDS with secondary pulmonary Mucormycosis; Our outcomes suggests the potential benefit of Remdesivir use in combination with Hydrocortisone and Monoclonal Antibodies in the management of severe COVID-19 ARDS in this group, as well as the importance of early administration of broad empirical antimicrobial coverage, including antifungal coverage, given the higher risk of secondary bacterial and fungal infections.

    These case series illustrate and hypothesize that patients with COVID-19 requiring mechanical ventilation in a prone position are at an increased risk for pressure ulcer development from the positioning changes process and the hyperinflammatory state intrinsic to the infection and require frequent skin surveillance. Preventative solutions to decrease shearing forces between the skin and devices should be considered and specific guidelines for prone positioning implemented.

Our case suggests the importance of more rigorous monitoring and aggressive correction of coagulopathies to prevent secondary sequelae of HLH which remain life threatening despite an improvement in the underlying disease process.

    We present an additional case of HSV-1 as an etiology of HLH.Case description: 66-year-old male with a past medical history of gastric adenocarcinoma status post total gastrectomy, on fluorouracil, leucovorin, oxaliplatin, docetaxel and trastuszumab recent diagnosis of typhilitis treated with levofloxacin and metronidazole presented with abdominal pain, diarrhea, and poor intake...He was started on antibiotics which included cefepime, vancomycin, and doxycycline...The patient was started on IV acyclovir and continuous renal replacement therapy given continued decompensation of renal function...Treatment would consist of etoposide, dexamethasone or methotrexate in addition to supportive care via blood product transfusions and treatment of underlying etiology. Literature review showed mostly pediatric case reports of HSV-1 induced HLH but rarely was it described in the adult population.

    Management of PAL remains challenging with lack of standard guidelines to define a systematic approach, Endobronchial valve (EBV) has proven to be an efficacious intervention in the management of PAL.Case: 29-year-old female non-smoker without significant past medical history was transferred to our hospital for VV ECMO cannulation secondary to acute hypoxemic respiratory failure due to SARS-CoV-2 pneumonia.Her hospital course was complicated by PE, Pseudomonas/Proteus bacteremia, acute renal failure requiring dialysis, and subsequently left-sided pneumothorax requiring chest tube insertion while on mechanical ventilation.The patient received treatment with dexamethasone for her SARS-CoV-2 and eventually had a tracheostomy placement...Different less invasive options have been tested, such as autologous blood patch, Heimlich valve, and albumin-glutaraldehyde tissue adhesives. EBV placement remains a more definitive and minimally invasive way to treat PAL and its utility in critically ill patients should be studied in prospective trials.

Steroid injections demonstrate promise in halting progression of pulmonary insult but require further research for its efficacy and utility in the treatment protocol.

    CD-1 male mice were immunosuppressed with cyclophosphamide (200?mg/kg) and cortisone acetate (500?mg/kg) on days ?2 and +3, relative to infection. Rhizopus infection causes lung tissue injury characterized by decreased AT2 cell markers and compromised endothelial cells during early infection consistent with clinical angioinvasion which likely contribute to bad prognosis.

    Through examination of 96 cases of srPJP, we have been able to identify more specific steroid dose thresholds and underlying conditions that increase the risk of developing this iatrogenic and often fatal infection. In particular, the use of dexamethasone in cancer patients and the presence of a sulfa allergy may be previously unrecognized barriers to prophylaxis.

    The presence of structural changes due to the previous tuberculosis infection led to diagnostic challenges as well portended a poorer prognosis in the patient. There wer several risk factors found for development of more severe residual pulmonary functional and radiological abnormalities 2 yeras after discharge from hospitals: older age ,obesity,kidney failure ,DM were associated with more extend severe residual lung abnormalities.Extend IMV and lack of use dexamethasone at the time of ICU admission and first 3 -months after discharge were associated extend fibrotic-like lung abnormalities with more severe pulmonary functional impairment.
