Methods 170 patients were enrolled in a Continuous Quality Improvement (CQI) project, including Baseline (N=72, with surgeon-placed intraoperative transversus abdominis plane (TAP) block using bupivacaine plus liposomal bupivacaine) and pilot (N=98, with preoperative bilateral aQLB using bupivacaine plus dexamethasone and methylprednisolone acetate) patients who underwent unilateral (n=72) or bilateral (n= 98) deep inferior epigastric perforator (DIEP) flap reconstruction either immediately (unilateral n = 49, bilateral n =69) or delayed (unilateral n=23, bilateral n=29) after mastectomy...There were no post-procedural thromboembolic events in either cohort. conclusions Pre-operative aQLB is an effective alternative to the conventional surgeon placed TAP block as an opioid-reducing pain control modality for DIEP flap reconstruction immediately after unilateral or bilateral mastectomy.

Surgical procedures may provide slightly better results than conservative treatments, but pain recurs in the long-term due to accumulation of scar tissue and increased inflammation.

There was no significant change in heart rate slowing between the dexamethasone and non-dexamethasone group, potentially negating the theory of a transient early binding of sugammadex to corticosteroids with later more avid binding to rocuronium and unbinding the corticosteroids, or, it may be that other corticosteroids, such as hydrocortisone are required to prevent this effect. Prospective trials evaluating this effect with cortisol concentration measurement are needed to further evaluate the hypothesis.

This study contained two experimental designs using the ICR mouse model for 1) determining the association between aging and gut microbiota (by analyzing murine fecal samples) and 2) determining the association between sarcopenia and gut microbiota in mice treated with microorganisms or dexamethasone...Although the sample size was small, these results point to similarities in the gut microbiota between aging and sarcopenia and to differences between young and old individuals. The results on gut microbiota obtained in this study form a basis for studying the development of sarcopenia in geriatric animal models in the future.
