    ERYTHRODERMIC BULLOUS PEMPHIGOID WITH NORWEGIAN SCABIES : A RARE CASE REPORT () -  Nov 6, 2023 - Abstract #ISDS2023ISDS_414;
    Recommendation treatment for NS is a systemic scabicides, single or recurrent dose of oral ivermectine depending on the severity of the case.We present the case of a 55-year-old female with a 4-week history of itchy erythematous plaques, developing into tense blisters, urticarial plaques, erosions and excoriations distributed symmetrically on her head, trunk, and limbs...Systemic methylprednisolone (125mg/day for 10 days) followed by oral methylprednisolone (32 mg/day initially), oral scabicides (ivermectine 12 mg single dose), and topical corticosteroids were administered. After 43 days of follow-up, tense blisters, urticarial plaques and erosions had subsided, and no new lesions were noticed

    Perceived stress can trigger psoriatic lesions in human skin in vivo, which can be suppressed by aprepitant () -  Nov 6, 2023 - Abstract #ISDS2023ISDS_325;
    Five days after psoriasis manifestation, we administered them with dexamethasone (DEX), for three days...These observations provide the first conclusive evidence that perceived stress can indeed trigger psoriatic lesions in human skin in vivo, likely at least in part via the induction of substance P-dependent neurogenic inflammation. Moreover, our data suggest that aprepitant, which is FDA-licensed as an anti-emetic, may be repurposed to suppress stress-induced psoriasis relapses/exacerbations.

    Journal, IO biomarker:  Glucocorticoids alleviate particulate matter-induced COX-2 expression and mitochondrial dysfunction through the Bcl-2/GR complex in A549 cells. (Pubmed Central) -  Nov 5, 2023
    The PM-induced increase in COX-2 protein, mRNA, and promoter activity was suppressed by glucocorticoids; this effect of glucocorticoids was antagonized by the GR antagonist RU486...Glucocorticoids regulate the PM-exposed induction of COX-2 expression and mitochondrial dysfunction and increase the interaction between GR and Bcl-2. These findings suggest that the COX-2/PGE2 pathway and the interaction between GR and Bcl-2 are potential key therapeutic targets for the suppression of inflammation under PM exposure.

    Journal:  The efficacy and safety of glucocorticoid stent implantation compared with oral glucocorticoid during perioperative period in chronic rhinosinusitis with nasal polyps (Pubmed Central) -  Nov 4, 2023
    during surgery, and patients in the oral glucocorticoid group received postoperative oral methylprednisolone at a dose of 0.4 mg/kg per day for 7 days, followed by a tapering of 8 mg per week to 8 mg followed by maintenance therapy for 1 week, for a total of 3-4 weeks...In the oral glucocorticoid group,1 patient experienced irritability, and 1 patient experienced weight gain. The glucocorticoid stent implantation has comparable effects to oral glucocorticoid in improving postoperative nasal symptoms, reducing nasal mucosal edema, scar formation, and nasal secretion in patients with CRSwNP undergoing FESS, with a better safety profile.

    Eligible patients in the steroid group received at least 1 dose (intravenous, intramuscular, or oral) of dexamethasone, hydrocortisone, methylprednisolone, prednisone, or prednisolone...This study suggests that steroids for irAE management may not abrogate irAE-associated survival benefits. However, early steroid administration within 2 months of ICI initiation is associated with shorter survival despite continuation of ICI therapy.

    Journal:  Myelin oligodendrocyte glycoprotein antibody-associated optic neuritis in a post-COVID-19 infection patient. (Pubmed Central) -  Nov 4, 2023
    While optic neuritis has been associated with both COVID-19 infection and vaccination, MOG IgG antibody-mediated optic neuritis is also a possible manifestation. This type of optic neuritis associated with COVID-19 infection does not show a similar pattern of frequent recurrences as seen in non-COVID-19 related optic neuritis.

    Journal:  A Novel Pathogenic variant in NR0B1 gene associated with Congenital Adrenal Hypoplasia (Pubmed Central) -  Nov 4, 2023
    On the physical examination, hypotension, hyponatremia, and hyperkalemia, as well as mucosal hyperpigmentation and bilateral cryptorchidism were observed, therefore, adrenal crisis was diagnosed, starting fluid resuscitation with saline solution, hydrocortisone, and fludrocortisone, which stabilized the patient...Con clusion: A novel pathogenic variant associated with X-linked adrenal hypoplasia is described. Variants in the NR0B1 gene should be a differential diagnosis in a male patient with the association of primary adrenal insufficiency and hypogonadism.

    Journal:  Successful antibiotic management of Staphylococcus epidermidis endophthalmitis after implantable collamer lens implantation. (Pubmed Central) -  Nov 3, 2023
    In antibiotic therapy, the excellent compliance and close follow-up of this endophthalmitis patient enabled careful postoperative surveillance on the effect of antibiotic therapy, avoiding the removal of the ICL or the loss of the integrity of the eye. The risk of potential infectious endophthalmitis post-ICL surgery should be fully emphasized during preoperative counseling.

    Journal:  Bronchodilator reversibility testing in morbidly obese non-smokers: fluticasone/salmeterol efficacy versus salbutamol bronchodilator. (Pubmed Central) -  Nov 3, 2023
    Obese subjects showed a high prevalence of restrictive patterns (23.4%), with non-significantly lower spirometric values compared to normal and overweight subjects (p?>?0.05). The magnitude of bronchodilation, as identified by spirometry, following fluticasone/salmeterol was higher in all participants, with a significant increase in obese subjects with a p-value of 0.013, 0.002, and 0.035 for FEV, FEV% predicted, and FEV/FVC, respectively.Fluticasone/salmeterol combination increases FEV, FEV% of predicted, and FEV/FVC ratio than the conventional test using salbutamol inhaler, and it can be a potential candidate for assessment of airway obstruction using reversibility test, especially among the obese population.
