    Journal:  Functional roles of CD26/DPP4 in lipopolysaccharide-induced lung injury. (Pubmed Central) -  Apr 24, 2024
    We previously reported that DPP4 inhibition by sitagliptin attenuates lipopolysaccharide (LPS)-induced lung injury in mice...In summary, this study demonstrated that genetic deficiency of Dpp4 attenuates inflammatory responses but not permeability in LPS-induced lung injury in mice, potentially through differential functional roles of CD26/DPP4 expression in resident cellular components of the lung. CD26/DPP4 may be a potential therapeutic target for ARDS and warrants further exploration to precisely identify the multiple functional effects of CD26/DPP4 in ARDS pathophysiology.

    Characterizing Outpatient Diuretic Therapy in Children With Bronchopulmonary Dysplasia (San Diego Convention Center, Area G (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_3978;
    Just under half of our study population was on diuretic therapy in the outpatient setting, with chlorothiazide being the most commonly prescribed. Subjects on diuretic therapy were more likely to be on supplemental oxygen in the outpatient setting, but diuretic status was not associated with age of weaning supplemental oxygen.
  • ||||||||||  Post-Operative Outcomes of ARDS after VATS: The Seismic Shift in ARDS Clinical Practice Guidelines (San Diego Convention Center, Area E (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_3587;
    Our case highlights the need for early recognition of ARDS and the shift in the guidelines, which has provided clinicians the scientific endorsement for the early use of corticosteroids in managing this challenging disease. This intervention is likely to be foundational in the propagation of more favourable ARDS outcomes of varying etiologies.

    Methimazole-Induced Hypothyroidism Presenting as Recurrent Thoracic Cavity Effusions and Ascites (San Diego Convention Center, Area C (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1512;
    This was evident in our case with the patient's history of dialysis dependence and hyperthyroidism from Grave's disease, which did not provide sufficient explanation for her recurrent serosal effusions. In the appropriate patient, thyroid function analysis is crucial in identifying medication-induced hypothyroidism as correction of thyroid hormone levels prevents future effusions.

    ACUTE RESPIRATORY FAILURE SECONDARY TO SUSPECTED NALOXONE-INDUCED PULMONARY EDEMA () -  Jan 4, 2024 - Abstract #SCCM2024SCCM_1964;
    Treatment is similar to sympathetic crashing acute pulmonary edema (SCAPE) with nitroglycerin boluses and high-rate infusions with non-invasive or invasive ventilation for airway support...While a vape-induced lung injury may have contributed to earlier respiratory symptoms, it has not been linked to SCAPE-like symptoms. NPE requires increased education for healthcare providers due to severity of illness.
  • ||||||||||  CAPILLARY LEAK SYNDROME CAUSED BY CALCIUM CHANNEL BLOCKER TOXICITY SUCCESSFULLY TREATED WITH ECMO () -  Jan 4, 2024 - Abstract #SCCM2024SCCM_1012;
    The mechanism by which CCB toxicity can cause capillary leak syndrome is not fully understood, but may be caused by selective precapillary arteriolar dilation and possibly L type calcium channel modulation of vascular endothelium permeability. This is a rare case of capillary leak syndrome resulting in distributive shock, ARDS, and severe end organ damage which was successfully treated with ECMO.

    This should be cautiously distinguished from 99mTc-pertechnetate uptake, considering that the salivary glands, oral cavity, and stomach were not visualized. Our case demonstrates that augmentation of blood supply, enhancement of capillary permeability, and accumulation of inflammatory exudate may be involved in the pathological process of Graves disease.
  • ||||||||||  Journal, Machine learning:  Machine learning decision tree models for multiclass classification of common malignant brain tumors using perfusion and spectroscopy MRI data. (Pubmed Central) -  Aug 24, 2023
    In hierarchical order of importance, the root node (Cho/NAA) in the decision tree algorithm was primarily based on the proliferative, infiltrative, and neuronal destructive characteristics of the tumor, the internal node (PSRmax), on tumor tissue capillary permeability characteristics, and the end node (Lac/Cr or Cho/Cr), on tumor energy glycolytic (Warburg effect), or on membrane lipid tumor metabolism. Our study shows potential implementation of machine learning decision tree model algorithms based on a hierarchical, convenient, and personalized use of perfusion and spectroscopy MRI data for multiclass classification of these brain tumors.
  • ||||||||||  Protein-Losing Enteropathy: An Isolated Presentation of Lupus Nephritis (Exhibit Hall) -  Jul 29, 2023 - Abstract #ACG2023ACG_3131;
    She had 1 prior hospital admission with a similar complaint and was diagnosed with a UTI, colitis, anemia, and unspecified fever, and was treated with a course of cefuroxime...Patient was started on high dose steroid taper, hydroxychloroquine, azathioprine, and lisinopril with subsequent improvement...Endoscopic biopsies aid in excluding other disease processes, but may not may not define the etiopathogenesis of protein-loss enteropathy, and in many cases, they are normal or show nonspecific findings. Early and aggressive steroid and immunosuppressant therapy is necessary to improve prognosis.
