    P=N/A
    Esketamine was used for anaesthesia induction and maintenance in the EKT group, and sufentanil and remifentanil were used in the OP group...Elderly patients receiving esketamine-based opioid-free anaesthesia had more stable hemodynamics, better postoperative analgesia, and reduced PONV incidence compared to those undergoing opioid-balanced anaesthesia. The trial was registered at the Chinese Clinical Trial Registry on September 1, 2023 (identifier: ChiCTR2300075324).
  • ||||||||||  Journal:  Managing Pin Tracts in Open Tibial Fractures: The Role of Medical Waste Rubber Bung. (Pubmed Central) -  May 5, 2025
    Group A exhibited a significantly lower rate of severe infection and prolonged time to initial infection compared to Group B (both P < 0.05). Group A also had a significantly lower rate of PTI at the tibial telangiectasia than Group B. The study underscores that compression in EF management is necessary to significantly reduce the incidence of severe PTIs, especially in the tibial metaphysis, and to delay the onset of initial infection among patients with open tibial fractures.

    While operative management may offer superior RTP to the preinjury level, the decision should consider potential complications and patient preferences. Standardized reporting of RTP outcomes is essential for future research to facilitate comparison and optimize management strategies.
  • ||||||||||  Journal:  Maternal Immune-Mediated Conditions and ADHD Risk in Offspring. (Pubmed Central) -  May 5, 2025
    These results suggest direct effects on fetal development through events at the maternal-fetal interface which may alter fetal immune responses and potentially lead to greater risk of ADHD in the offspring. Asthma may be a possible exception to this mechanism, as paternal asthma was also linked with risk of offspring ADHD.
  • ||||||||||  Journal:  Severe long-term clinical sequelae among Sudan ebolavirus disease survivors 2 years post-infection. (Pubmed Central) -  May 5, 2025
    Conclusions This study demonstrates that SUDV survivors develop long-term clinical sequelae characterized by persistent multi-systemic clinical symptoms. Detection of viral RNA in semen and breastmilk for up to 7 months post-infection suggest prolonged persistence, with the possibility of latency and reactivation of the virus.
  • ||||||||||  Journal:  Atypical cause of radiating leg pain: Visceral referred pain due to ulcerative colitis. (Pubmed Central) -  May 5, 2025
    The pathophysiological mechanism behind visceral referred pain originating from the distal colon -a region potentially affected in inflammatory bowel diseases such as UC- is likely due to the convergent input of somatic and visceral (lumbar splanchnic) afferent fibers at the L1-L2 level of the lumbar spinal cord, causing referred pain to the L1 and L2 dermatomes, corresponding to the groin and thigh. In this case report, we aim to underline the possibility of referred visceral pain as an uncommon etiology in unexplained radiating leg pain and the importance of thorough medical history and diagnostic evaluation.

    Surgical intervention, by total knee replacement with adjunctive measures to stabilize the fractures, can effectively alleviate pain and improve functional outcomes in these patients. V.
  • ||||||||||  Journal:  Pelvis/acetabulum: management of geriatric injuries. (Pubmed Central) -  May 5, 2025
    Despite advancements, controversy remains regarding the optimal treatment of complex acetabular fractures in elderly patients. This review synthesizes the latest evidence and expert perspectives to aid clinicians in selecting the most appropriate management strategies, with the goal of restoring mobility, minimizing complications, and enhancing the quality of life in this vulnerable population.
  • ||||||||||  Journal:  Trauma systems: a global comparison. (Pubmed Central) -  May 5, 2025
    Other countries, such as the United States and South Africa, face different challenges linked to disparities in quality of and access to care between public and private systems. Although centralization of trauma care, standardization of national trauma care systems, and investment in workforce and infrastructure are universal goals for improving outcomes, solutions tailored to each country are required to optimize trauma systems globally.
  • ||||||||||  Journal:  Current concepts in tibial plateau fracture management: a Spanish Orthopaedic Trauma Association review. (Pubmed Central) -  May 5, 2025
    With the classification of columnar involvement and their variants, surgeons can use computed tomography scans to formulate solutions to approach and fix individual fracture patterns. Finally, a finite element analysis may inform the surgeon's decision for immediate weight bearing of a split plateau fracture after fixation with locking screw plates or cannulated screws.
  • ||||||||||  Journal:  Proximal humerus fractures: Treatment controversies. (Pubmed Central) -  May 5, 2025
    Hemiarthroplasty may not provide good, reliable function but may be the best option in younger or middle-aged patients when fracture fixation is not feasible. While several studies reported no benefit of surgical treatment in elderly patients, more recent literature showed better outcomes with reverse total shoulder arthroplasty.
  • ||||||||||  Journal:  Spontaneous Orbital Emphysema: A Case Report. (Pubmed Central) -  May 5, 2025
    Spontaneous orbital emphysema, while rare, should be considered in patients with sudden periorbital swelling without history of trauma. Conservative management is often effective, but vigilant monitoring is crucial.
  • ||||||||||  Journal:  Redesigning to V-Y Fasciocutaneous Flaps: Optimizing Outcomes in Patients With Pressure Injuries and Spinal Cord Injuries. (Pubmed Central) -  May 5, 2025
    Interestingly, our approach records complication and recurrence rates equal to those of other flap types reported in the literature, with the advantage of not having full or partial necrosis. We describe an easy, highly reproducible, new technique of marking, tracing, dissecting, and advancing the V-Y fasciocutaneous flap so that its simplicity makes it the preferred choice of local flaps for this type of lesion; additionally, this technique is applicable to defects of varying sizes in any anatomical region.
  • ||||||||||  Journal:  Effect of plastic deformation on the hydrogen embrittlement of ferritic high strength steel. (Pubmed Central) -  May 5, 2025
    The increased hydrogen uptake did not lead to a decrease in fracture strain, which remained between 6 and 10% for all pre-strains, compared to 60% for full SSRT tests that were charged for a shorter time. This research highlights the necessity of high plastic strains and the presence of hydrogen in the environment during crack growth to cause HE in ductile steels.
  • ||||||||||  Journal:  Flexural Strength and Stress Analysis of CAD-CAM Milled Glass Fiber Post and Core. (Pubmed Central) -  May 5, 2025
    Intermediate values were demonstrated by conventional posts. In comparison, to the customized post, the stress study revealed that conventional and CAD-CAM posts had a lower stress in the apical area and a lower general root stress value; but the cervical stress from CAD-CAM posts were practically twice of conventional and customized posts.
  • ||||||||||  Journal:  Care Utilization and Measures of the Subjective. (Pubmed Central) -  May 5, 2025
    The finding that statistical groupings of patient personal factors were not associated with planned utilization of visits, tests, or treatments is inconsistent with other evidence linking mindsets actual utilization. A better understanding of sources of variation in planned and actual utilization is needed to help limit unwarranted variation and enhance effective use of resources.
  • ||||||||||  Journal:  Forces on the Incisor Teeth During Odontoplasty of the Cheek Teeth in Sedated Horses. (Pubmed Central) -  May 5, 2025
    Forces were recorded for different incisor separation distances and for dental rasping at a defined incisor separation distance. The results showed a significant difference in the median loads on the incisors at 82 mm (198.88 N), 92 mm (214.18 N), and 102 mm (293.95 N) incisor separation distance, and between active treatment with a mechanical bur (173.28 N) and no active mechanical bur (237.81 N) with maximum peaks up to 3783.60 N. Increasing bodyweight and mandible length showed a correlation with increasing forces, whereas age, gender and regularity of dental treatments did not suggest any influence on force development.
  • ||||||||||  Journal:  Stabilisation of thoracolumbar vertebral fractures and luxations in cats with a polyaxial screw/rod system. (Pubmed Central) -  May 5, 2025
    The results showed a significant difference in the median loads on the incisors at 82 mm (198.88 N), 92 mm (214.18 N), and 102 mm (293.95 N) incisor separation distance, and between active treatment with a mechanical bur (173.28 N) and no active mechanical bur (237.81 N) with maximum peaks up to 3783.60 N. Increasing bodyweight and mandible length showed a correlation with increasing forces, whereas age, gender and regularity of dental treatments did not suggest any influence on force development. The polyaxial screw rod system described here is a potential option for stabilization of thoracolumbar vertebrae in cats.
